It could be any of the three reasons you listed. People often do fake mental illness in order to get off easy. But forensic psychologists are trained to determine if they are faking or not. He could also be extremely exhausted if he didn't sleep in jail. Or they could have given him Xanax or Klonopin (anti-anxiety medication) to calm him in down in order to take him into the court room. His eyes are bug-eyed, and his pupils might be dilated, and he keeps nodding off for a split second. If so, then he would probably be on medication.
3 He was acting
He was in the top of his class.He knows just how a crazy person would act.He already knows what questions will be asked by the psychologist.
Faking a mental illness and knowing just how to do it.
Perhaps the reality of it all is sinking in. Maybe the drugs wearing off? Maybe he realizes that his "joker" days are over and that the end is near.
I believe it is a combo:
He has not slept (much) since the murders
he has been drugged by jailers due to his beharior in prison
he is on sensory overload
