1> school leaving / berth (certificate with date and place of birth mentioned)
2> light AND telephone bill (present and last years of same month)
3> bank statement (last 6 months only) no true copy required
4> Ration Card / Pan Card / Driving Licence / Voter ID (any one)
5> NOC from your building / LL agreement from the owner of house

Yep You will need Birth Certificate if you popped out after 1990. But its good to get one made by youe district registrar.
Also you will need a residential proof.. say a rent agreement with Electricity Bill with Police verification.
Bank statement and Pan Card are conditional generally they discard these proofs.
Go to local Passport office there help desk are generally very helpful.. Also you can contact some Dalal to get it done for you. Just don't fill form with half documents or you will run a lot.
Bank Statement 10th marks card is fine, I don't think they accept Pan card as a proof,Latest Bank Statement with Seal and signature of the manager and updated passbook with your photo in it will do i guess.Easiest way to apply is with these Documents 10th marks card, Voters Id and a DL if your born after 1988 u'll need your Birth Certificate which is mandatory.
U would also need (1) residential proof & (2) Birth certificate
