Fake.
There will be a lot more blood. There are 5-6 Litres in the human body. The man would have a lot of venous blood loss if he was cut a lot.
He did not struggle much. A reason for not struggling is the fainting response (from a lot of blood loss). But this video clearly shows that not a lot of blood was spilt. For a fainting response to occur quickly, an artery would probably be cut and blood will be spurting out in intervals (veins just flow). He did not really fight (no adrenaline in him), so it appears really fake. Another person would be flailing his arms, or shooting punches, or kicking, or trying to run. A good reason to not do this is if they are fainting, but as I said, no blood spilled.
It appears very unrealistic the fact that a man would close the door of his car and get very close to the tiger. Maybe he thought his retro camera had zoom, and thought he was further away than what he thought through the camera. But that is pretty unlikely. Plus he went in so deep that a lion ambushed him from behind. That is either exceptionally unfortunate or just exceptionally unlikely. Tragic things like this happen, but this one seems very, very unlikely.
I think the give away factor for what makes this seem very fake is the fact that the camera focuses on a crying baby. The baby isn't looking out the car, how does he know something bad is happening? By the look of the baby, he looks old enough to only see 1 feet (it is how babies develop, at first their vision is 30 cm). It is also dramatic how at one point it just focuses on the people crying. It is like it is rehearsed, part 1, man walks into lions, part 2, everyone is shocked and crying, part 3, security comes in to take away body and crying people.
The biggest give away factor is the dozens of camera angles. This 'cameraman' is either lightning fast at running around the lions, and zooming in, or there are a lot of 'cameramen' with equally as shaky hands, or it is fake.
It sure looks a lot more real than a similar one I saw on the internet from the mid 70ies. In the one I saw German tourists were videoing male lions & the guy crawled onto the roof of a car & the lions sat on him and devoured his intestines while he just looked in shock as his wife & baby screamed.
In that one though the camera was held very still & no one tried to do anything. But both this & the other one I saw seem real.

well holy **** it looks very real to me. especially the way the lions are being aggressive and territorial to "their" food towards some of the other lions. woah..
