Badly maintained nuclear reactors are an accident waiting to happen & it did.
First of all, the Russians put their reactors in warehouse-like buildings where the sides are designed to open to the environment in case of an emergency.
The Russians ran a test on the reactor right after a shutdown and restart. This changes the dynamics in the core. American reactors are not allowed to restart for a few days after shutdown. Chernobyl restarted after about an hour of downtime. The control rods were pulled completely out of the core (not possible in US reactors except at refueling time when boron is added to the water to take their place). When it was realized that the reactors were behaving "badly" the Russians tried to reinsert the control rods. That, too, had a problem in that the first foot of Russian control rods actually added reactivity to the core instead of reducing it. The reactor overheated which turned the external cooling system into steam which caused damage to the reactor. Russian reactors of that type used a graphite shield to moderate (slow down) neutrons to the speed suitable for nuclear reactions. The graphite caught fire which only added to the damage.
We will never get a good account for the number dead because the Russians have sealed the records. They would not even admit that they had an accident until we released information on the size of the core, its initial enrichment, and the extent of fuel burn-up. We were able to calculate all this from airborne isotopes measured at the Three Mile Island plant in Pennsylvania.

Doses: A lot of the first responders received doses in excess of 500 REM (guaranteed to cause 100% death in a few hours). As you moved away from the site, the doses were reduced. We calculated that many had 300 REM (50% mortality)
By comparison, American reactors are housed in buildings designed to contain the results of any type of accident. When Three Mile Island had its accident March 29, 1979, the dose to the nearby community had to be calculated because it was too low to be measured.
